I'm just making an assumption here, but use Entity to rip the BSP in .OBJ format, which you can import into 3D Studio Max. Save it using a model exporter as a .JMS. Once that's done, you'll need to compile it with Tool into a BSP for Halo CE, you can find more information on tool by running CMD in the HCE root directory and typing in "Tool." Should tell you how to compile it, it'll need to be in Halo Custom Edition/Data/Levels/YourLevelNameHere for it to compile properly, it'll show up in Tags/Levels/YourLevelNameHere. Then with the BSP imported, open the .scenario tag in Guerilla and add a skybox. Then open it in sapien, put in all the level data. Save it, compile it into a .map, play.
I'm not sure how the materials (textures) will work, I haven't messed with Halo 2 models enough.

Doesn't H2V lack a bitmaps.map for Entity to work?

Only way would be to find the person who made the map and ask them for the max file.
