Removing Phantom BSP is very time consuming. Eliminating all nearly coplanar warnings is a good start. After that there will still be phantom bsp even though tool creates the structure with no errors or warnings. In sapien there is a command, I forget the exact name but it is something like structure_bsp or structure_phantom_bsp. When flying through Sapien will show the collision properties of every triangle and phantom bsp. You can then use your targeting to find the 3 points of the triangle and return to your model and move those vertices one at a time until the phantom bsp goes away. Flying through Sapien looking at almost every potential perspective is time consuming, but knowing that you've eliminated all the phantom geometry can be worth it.
