building intermediate geometry...

building collision geometry...
reducing collision geometry...
building collision bsp...
### ERROR found z buffered triangles (red).
### ERROR couldn't build bsp because of overlapping surfaces (orange).
### ERROR couldn't build bsp2d.
### ERROR couldn't build leaf.
### WARNING found nearly coplanar surfaces (red and green).
### ERROR failed to import collision bsp.

^^^ This means nothing to me what do I need to do?

The z buffered triangles you can ignore, although you should try to fix them.
The overlapping surfaces is what stops it from making it all the way though.
Import the .wrl and look for orange faces. If you ripped a BSP and brought it back into max, check the double sided surfaces, they have faces on both sides and tool doesn't like that.

OK it's my own BSP so double sided faces wouldn't be the problem. What's this .wrl file, and what do I do with it?

You import it, and double sided faces are the only reason that didn't get though tool. The rest shouldn't have stooped it.

He doesn't have any open edges. He has some duplicated double sided surfaces.

No he has overlapping faces.
Import the nameoflevel.wrl and uncheck the 3 boxes, find the orange part (this is showing where the problem is) once you know where the overlapping faces are then hit delete to delete all the wrl stuff.
Delete both or one of the overlapping faces and "create" new ones.

Normally, you only get that with duplicate double sided surfaces...
Well, at least I do anyway.
