It means sapien wont load until you rename or delete that dll. I would suggest renaming it since you need that dll for Adobe CS5 products. You can always write a batch file that renames the dll, opens sapien, waits for sapien to close, then renames the dll back.

example:
rename %systemdrive%\windows\system32\msi.dll %systemdrive%\windows\system32\msi.dll.temp
sapien.exe
rename %systemdrive%\windows\system32\msi.dll.temp %systemdrive%\windows\system32\msi.dll

This would be ran from your HCE folder as administrator. It would rename the dll, start sapien.exe linked to the CMD window, as long as CMD is still open when sapien is closed, it will rename the dll back.

are you running any adobe products at that time, which may be recreating the file?
